What should A do?

Scenerio:
A and B are couples of 4 years. For the past 2 years, A suddenly got quite sick and body is starting to fail.A predicted that she could go on for at the most 3-5 more years. At this point of time, A cannot decide whether to let B go and allow B to find her new world out there or just hold on to B until she dies.Dont you all think that A would be too selfish if she keeps holding on to B?

Note: A & B are not young anymore.
